La simulación de alta fidelidad clínica con actuación y la simulación virtual en metaverso como estrategias para la formación de futuros médicos.
Resumen
Introducción: La simulación clínica de alta fidelidad con pacientes estandarizados y la simulación virtual en entornos inmersivos como el metaverso han emergido como estrategias educativas complementarias. Metodología: Se realizó una revisión sistemática cualitativa de la literatura según PRISMA 2020. La búsqueda se efectuó en Medline (PubMed), Scopus y Google Scholar (2020-2025). Se utilizó la pregunta PICO para estructurar la búsqueda, con términos MeSH como "high-fidelity simulation", "standardized patients", "metaverse" y "medical education". La calidad metodológica se evaluó mediante la escala de riesgo de sesgo de Cochrane y los niveles de evidencia SIGN. Resultados: Se identificaron 4301 registros, de los que 7 cumplieron los criterios de inclusión. Ambos enfoques demostraron eficacia en el desarrollo de habilidades técnicas y no técnicas, aunque con fortalezas diferenciadas. La simulación con actuación destacó por su capacidad para entrenar destrezas clínicas prácticas, toma de decisiones bajo presión y habilidades comunicativas, mientras que la simulación en metaverso mostró ventajas en accesibilidad, escalabilidad, repetición del aprendizaje y visualización inmersiva de procesos fisiológicos complejos. La evidencia presentó un riesgo de sesgo bajo a moderado, con un nivel de evidencia 1+ a 2+ según SIGN. Conclusiones: La simulación con actuación constituye una herramienta altamente efectiva para recrear escenarios clínicos complejos con realismo emocional y espontaneidad, aspectos difíciles de replicar mediante tecnologías digitales actuales. En contraste, la simulación en el metaverso ofrece beneficios en términos de accesibilidad, escalabilidad y repetibilidad del aprendizaje. La integración de ambas metodologías representa la estrategia más adecuada para la educación médica contemporánea, permitiendo una formación integral que combine el desarrollo de competencias clínicas con la sensibilidad humana y el aprovechamiento de la innovación tecnológica. Se proponen implicaciones educativas concretas para el diseño curricular híbrido.
